Humana Inclusa is seeking a Float Case Manager to support Wisconsin Family Care members in the SW Pod 2 region (Trempealeau, Jackson, La Crosse, Monroe, Clark). You will cover caseloads for team members on leave, coordinating with Field Care Nurses and delivering person-centered care in homes and communities.
You will conduct comprehensive assessments, create MCPs, arrange non-medical supports, and document care with a focus on cost-effective services and member well-being.
