Proforma Health is seeking a Licensed or IT Therapist to join our Family Centered Treatment (FCT) Team serving multiple counties in Wisconsin. The role delivers clinical treatment to children and families in home and community settings.
You will manage a caseload of 3–5 families, collaborate with schools and community partners, complete documentation and treatment plans, and participate in weekly supervision and team meetings. Evening appointments and caregiver coaching may be required.
