Position: PK-12 Physical Therapist .3 FTE
Location: Bayfield

Physical Therapist

The Physical Therapist reports to the Director of Pupil Services and is responsible for the evaluation, program development, delivery of physical therapy to eligible students, evaluation of student progress, program evaluation, consultation and training for staff and parents, and liaison services with medical and other outside service agencies.

Essential Job Functions:

  • Conduct evaluations consistent with district physical therapy assessment protocol, prepare written reports of the evaluation findings.
  • Participate in meetings:
    As a member of the IEP team; as a member of the therapy staff (required staff meetings); and required district meetings.
  • Participate in the development of IEP for children found to be eligible for special education; help determine if therapy services are necessary for the child to benefit from special education; provide IEP related service information for IEP development.
  • Develop and implement an intervention plan consistent with the IEP; intervention plans are updated immediately following IEP meetings when therapy services are continuing. Child's progress reporting is provided consistent with IEP and in collaboration with teachers (for ages 3-21); discharge summaries are written when therapy services are discontinued.
  • Communicates with the IEP team on a consistent basis to collaborate and plan for a child's needs and progress. Integrates services into a child's natural environments and typical routines/activities in collaboration with other team members.
  • The model of intervention imbeds services into typical school, family, or community environments and activities and rarely utilizes a 'pull out' model.
  • Provides timely documentation/billing information for services provided: billing and related documentation are completed within seven days of the 'end of month' cycle; monthly attendance and expenses are documented on accounting form.
  • Moves and handles children and equipment as detailed in the Physical Requirement section of this job description.
  • Provides timely information and response to the Director of Special Education / Pupil Services as requested. Adhere to the ethical standards of the profession and participate in professional growth activities and continuing education opportunities. Adhere to established rules, regulations, and laws, and works cooperatively to accomplish the goals and objectives of the local education agency.

Environment/Travel/Equipment:

Travels to locations consistent with the IEP. This role routinely uses standard office equipment such as personal computers, printer, copy and fax machines, and telephone.

Knowledge, Skills, Abilities:

Current certification in CPR and First Aid and experience in school-based physical therapy is strongly preferred Valid driver's license Ability to work during non-standard work hours and travel

Certification/License Requirement:

State of Wisconsin Physical Therapy license from the Department of Safety and Professional Services Wisconsin Department of Public Instruction license Motor vehicle operator's license or ability to provide own transportation

Education:

Degree in Physical Therapy (BS, MS, Doctorate in Physical Therapy) from a school accredited by the American Physical Therapy Association.

Physical & Mental Requirements:

Constant (67-100%) bending, pushing, pulling, lifting, walking, squatting, reaching and stair climbing in order to perform the essential job functions. Lifting: 0 50 pounds constantly (66-100%) Over 50 pounds occasionally (1-33%) FLSA Status:

Range $18,919 - $29,823 (based on years of licensed experience). Benefits:
