• Conduct resident assessments and participate in developing and updating individualized care plans
• Administer medications and treatments as prescribed; document administration accurately
• Monitor residents’ conditions, respond to changes, and escalated as needed
• Collaborate with an interdisciplinary team to coordinate care and ensure smooth transitions
• Maintain complete, accurate documentation in resident records; ensure regulatory compliance
• Uphold safety, infection control, and patient-rights standards; provide patient and family education as appropriate

• Active RN license in Wisconsin or eligibility to practice in Wisconsin
• Ability to work in a long-term care/assisted living setting
• Willingness to participate in a flexible travel assignment
• Strong assessment, documentation, and communication skills
• Prior LTC experience preferred
