* ** JOB SUMMARY
** The Music Therapist provides music therapy that is designed to meet the patient’s psychological and/or physical needs.
* ** DUTIES & RESPONSIBILITIES
*** Performs patient assessments to evaluate appropriateness for a music therapy program and contributes to the plan of treatment.
* Reports changes in the patient’s conditions and needs to the Registered Nurse Case Manager or Director of Social Services.
* Observes for safety hazards in the patient’s and organization’s environments and takes appropriate steps to reduce or eliminate them.
* Designs and implements a variety of music therapy and plans to meet the individual needs of each patient.
* Collaborates with members of the Hospice Team in planning sessions.
* Provides therapeutic interventions.
* Provides education to the staff and community on the music therapy program.
* Maintains continuing education through academic studies, seminars, and workshops.
* Attends and participates in in services, staff meetings, and case conferences as required.
* Completes all documentation in an accurate and legible manner.
* Complies with hospice policies and procedures, laws and regulations, and standards of practice.
* Performs other related duties as assigned.

* ** JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
*** Graduate of an accredited music therapy program.
* Current certification from an accredited music therapy program.
* The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.  Must have and maintain a valid driver’s license, maintain automobile insurance coverage, and have access to an automobile.
* One year experience working in a health-related setting preferred.
