Position:
RN - Home Health and Hospice Department - Per Diem position
Location:Dodgeville, WI
Role & Department:Registered Nurse in our Home Health & Hospice Department
Role Responsibilities:- The Registered Nurse will provide direct in-home nursing care for home care and hospice patients
- Will communicate with physicians and other medical team members as is appropriate for the plan of care
- Will respond to on-call needs of home care and hospice patients
- Will provide direction and oversight for the work of LPN's and CNA's who care for their patients
- Will be responsible for assessing the needs of each patient assigned
- Planning and preparing the nursing care plan and then following through with that care plan
- Care for the patient includes the evaluation of the spiritual, emotional and physiological needs
- Will properly document patient care services performed in the electronic medical record
- Graduate of an accredited school of nursing
- Candidates must be licensed and hold current registration as a Registered Nurse in the State of Wisconsin
- Prior nursing services experience is preferred
- Prior home health care experiences are very desirable
- Ability to communicate well verbally and in writing
- Ability to plan, direct and/or implement and evaluate the nursing care delivered to patients
- Interest in professional growth, continuing education and participation in professional organizations is preferred
