Location: Upland Hills Health - Dodgeville Hospital Campus
Role & Department: Nurse Practitioner (APNP) that will work in the Continuous Care Services Department
Hours & Shift: Full-time (1.0 FTE) day shift position, Monday through Friday 8 a.m. to 4:30 p.m.
Role Responsibilities:The Palliative Care Nurse Practitioner is a professional nurse practitioner conducting Palliative Care inpatient, outpatient, nursing home and home visits
The Nurse Practitioner will also conduct face-to-face hospice certification visits in the home setting as needed
The Nurse Practitioner provides professional services to patients of all ages in accordance with Upland Hills Health’s practices and which reflects Palliative Care and Hospice philosophy and standards
Work with primary care providers to establish and maintain appropriate care to meet the needs of the patients
Provide face-to-face and clinical visits with hospice patients as needed
Assessments of hospice patients are required for hospice recertification as needed
Keeping records, appropriate charting and generating reports as necessary
Appropriately manages pain and other distressing physical symptoms of disease, illness or treatment in a timely manner
Demonstrates the ability to understand and communicate patient/family wishes as goals of care changes
Provide end of life education to patients and their families with compassion and respect
Demonstrates ability to support patient approaching last days of life and support family members through this process
Assesses and identifies psychosocial and spiritual needs appropriately
Reports findings to the primary care provider, continuous care services director as well as other appropriate medical providers and caregivers as needed
Attend in services and training sessions as needed to maintain credentials and stay up to date on processes and procedures
Assists with monitoring of Performance Improvement activities to ensure desired outcomes are achieved and maintained
Must comply with all applicable rules and regulations including the most current Palliative Care guidelines, HIPAA, current Hospice Conditions of Participation and Upland Hills Health policies and procedures
Attend hospice interdisciplinary meetings as needed
Completes other duties and tasks as necessary to provide excellent patient care while meeting departmental and organizational goals
Exceptional listening and communication skills with ability to convey information clearly, concisely and logically both verbally and in writing
Good analytical skills to assess specific needs of patients and adjust care, instruction, or interactions with patient
Ability to maintain confidential information
Qualifications:- Master's Degree in Nursing
- Have a current and valid Registered Nurse license and licensed as an Advanced Practice Nurse Prescriber (APNP) from the State of Wisconsin
- Current DEA number
- Must be board certified
- 3-5 years experience in Family Practice setting as a Nurse Practitioner
- Palliative Care experience highly preferred
- Chronic Care Management and Hospice background also preferred
- Prior experience using the EPIC medical record system is preferred but not required
- Previous experience with the Microsoft Office Suite software programs is beneficial
