Location: Chilton

Flexible work schedule Monday - Friday, with paid evening on‑call shift every other week and every 7th weekend. Required to work 2–3 holidays per year. Excellent wage and benefits, including participation in the Wisconsin Retirement System.

Overview

Under general supervision of the HHC/Hospice Supervisor, the nurse provides specific, comprehensive, and intermittent nursing services to ill, disabled, or dying patients in their homes and facilities. Plans and implements nursing care and performs related duties as required.

Essential Duties and Responsibilities
  • Evaluates assigned home health and hospice patients and identifies problems/needs, including physical, emotional, psychological, environmental, and educational.
  • Develops, communicates, and coordinates a goal‑oriented plan of care for each patient in collaboration with the patient, MD, family, and the hospice interdisciplinary group.
  • Implements plan of care, nursing interventions, patient/family teaching, and other procedures; delegates appropriately and ensures documentation of interventions.
  • Evaluates each patient’s response and modifies the plan of care based on signed physician orders.
  • Maintains strict confidentiality with client information.
  • Communicates effectively and timely with all members of the health care team; maintains up‑to‑date and accurate documentation of care provided.
  • Promotes and maintains health of families through teaching, counseling, and initiation of preventive, rehabilitative, or palliative measures. Educates patients and families regarding disease management, home safety, medication safety, end‑of‑life care, and anticipated outcomes.
  • Provides nursing services in the home and facilities for critically, chronically, and terminally ill patients under a medical plan of treatment established according to agency policy.
  • Coordinates services with other professionals and service providers and the hospice interdisciplinary group to assure a holistic approach to patient and family needs.
  • Plans, manages caseload, and provides nursing services within a specific geographic area.
  • Teaches, demonstrates, supervises, and evaluates services provided in the home by other agency staff.
  • Prepares and submits records and reports according to agency policy.
  • Assumes responsibility for self‑development and participates in appropriate continuing education activities to maintain competence within the scope of practice.
  • Participates in developing and implementing a community disaster plan.
  • Assumes administrative responsibilities as designated by the HHC/Hospice Supervisor.
  • May be assigned to perform on‑call duty.

Supervisory Responsibilities

This position has no supervisory responsibilities but contributes to oversight and guidance of HHC/Hospice Agency Aides and Licensed Practical Nurses in accordance with organizational policies, procedures, and applicable laws. Provides mentorship to nursing students.

Qualifications
  • Education and Experience: Graduate of a school of nursing accredited by the national professional nursing accrediting program, with an Associate’s Degree, Diploma, or Bachelor’s Degree in Nursing; two to three years of experience in a hospital, long‑term care facility, home health care, or hospice agency; or an equivalent combination of experience and training.
  • Language

    Skills:

    Ability to read, interpret, and document care and treatment, and to write routine reports.
  • Mathematical

    Skills:

    Ability to perform basic calculations, compute rates, ratios, and percentages.
  • Reasoning Ability: Ability to solve practical problems, interpret instructions, and draw valid conclusions.
  • Computer

    Skills:

    Knowledge of word processing and documentation systems.
  • Certificates, Licenses, Registrations: Current licensure in Wisconsin as a Registered Nurse; current Wisconsin driver’s license with dependable automobile and minimum insurance; CPR certification.
  • Other

    Skills and Abilities:

    Comprehensive knowledge of nursing process; current practice; use of community resources; interprofessional relationships; home health care principles and hospice philosophy; ability to apply scientifically sound theory in nursing practice.
Physical Demands

Regular use of hands, reach, standing, walking, bending, and lifting up to 25 lbs frequently and occasionally over 100 lbs. Requires hand‑eye coordination, color vision, distance vision, peripheral vision, and ability to adjust focus. Must wear gloves, masks, and other protective equipment.

Work Environment

Exposure to weather conditions, driving hazards, unclean indoor environments, and challenging interpersonal interactions. May encounter odors, infections, contagious diseases, toxic agents, temperature variations, dust, and blood‑borne pathogens. Noise level is usually quiet to moderate.
