Social Worker Designee – Norseland Nursing Home Westby, WI $21.25 - $24.97 hour

What You’ll Do Provides process for resident admissions and readmissions that include:
Coordination with nursing service
Complete and accurate gathering of resident information and timely dissemination of same to departments that need to know.
Payer source to be identified.
Procedures for admission at any time of day Provides resident advocacy and develops advocacy roles for staff
Maintains a close working relationship with the Director of Nursing to better ensure the delivery and coordination of quality care and service to the residents.
Assists residents and/or families with financial and legal matters. Be aware of Medical Assistance, Medicare Advantage Plans, and Medicare policies and criteria for eligibility. If necessary, referrals will be made to appropriate agencies in cases which the facility cannot serve.
Processes Level I and Level II screens timely, route and log information conjunction with nursing, completes resident POLST forms.
Perform general clerical duties including managing incoming and outgoing mail.
Schedule Options Monday - Friday schedule, 40 hours per week. Primarily business hours.

Qualifications
Graduate of an accredited university with a minimum of a BA or BS in Social Work or similar discipline.
Skilled nursing or long-term care experience preferred (not required)
Strong communication and teamwork skills
Compassionate, patient-centered approach
