Per Diem Hospice Registered Nurse

We are a Hospice Agency seeking a compassionate, dynamic, energetic, organized and efficient Per Diem Hospice RN for our Tri‑Cities location serving the surrounding areas. This role requires a registered nurse who values hospice care and will provide emotional support to help patients live their final days with dignity and comfort. You will conduct supervisory visits, provide feedback, participate in interdisciplinary meetings, and develop, review, and revise patient/family care plans.

• Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with Advisa Care Hospice Policies and Procedures, Medicare compliance.
• Consult with and educate the patient/family on disease process, self‑care techniques, end‑of‑life care, and ethical concerns.
• Supervise paraprofessionals providing services to the patient.
• Prepare clinical and progress notes; participate in IDG conferences.
• Participate in hospice performance improvement program.
• Maintain high level of customer satisfaction with our clients, families, and caregivers.
• Communicate information using current process and technology within organization.
• Position requires on‑call responsibilities.

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily and weekly office meetings as indicated.
• Ability to adapt to patient status and needs on a full time on-call basis.
