Encompass Health – Registered Nurse
Encompass Health is seeking a Registered Nurse to join our Healthcare & Medical Services team in an inpatient rehabilitation setting. In this role, you will deliver high-quality, patient-centered nursing care, coordinate and implement individualized rehab plans, and collaborate closely with an interdisciplinary team. You will monitor patient progress, educate patients and families, and ensure compliance with clinical standards and safety protocols.
Key Responsibilities- Supervise care and treatments, lead patient assessments, tailor care plans, and address patient concerns to provide exceptional care.
- Build meaningful relationships with patients and their families, understanding their unique needs to facilitate their recovery.
- Collaborate and communicate effectively with various departments to ensure coordinated care and provide direct patient care when needed.
- Serve as a resource for fellow staff, supporting them with complex patient needs and inquiries.
- Celebrate victories and milestones achieved by our patients.
- Valid RN licensures as required by state regulations.
- CPR certification (ACLS preferred).
- CRRN certification preferred.
- One year of experience in a rehabilitation hospital setting is preferred.

Performs appropriate assessment of physical, social, and psychological status. Seeks and evaluates information acquired from other members of interdisciplinary team. - Plan:
Uses assessment information and critical thinking skills to collaboratively develop individualized plan of care. Actively seeks patient, family, team, and physician involvement to develop plan of care including needed education. - Implement:
Directs the interdisciplinary care team via delegation, coordination, and collaboration. Provides or delegates care consistent with plan of care, guidelines of care and professional licensure provisions. - Evaluate:
Evaluates patient response to interventions through review of achievement of goals, clinical outcomes, patient, and family satisfaction. Revises plan of care as indicated and reassesses changes as appropriate. - Professionalism:
Promotes nursing profession and participates in development of others.
- Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate.
- Deliver skilled nursing interventions and symptom management aligned with goals of care.
- Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identifying.
- Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners.
