GENERAL RESPONSIBILITIES:
The CBRF Nurse provides direct and comprehensive nursing care in a Community Based Residential Facility (CBRF) under supervision of the registered nurse and/or designated supervisor.
STANDARD QUALIFICATIONS:
All employees of Stockbridge Munsee Community must meet the following qualification:
1. Must submit to and pass a pre-employment drug and health screening.
2. Must be at least 18 years of age, unless otherwise stated on job description
3. Must maintain an acceptable departmental attendance record
4. Must be able to work weekends, nights, and holidays
5. Must obtain and maintain Elder/Youth License as stated on job description.
DUTIES:
1. Provide high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures.
2. Complete basic nursing procedures (e.g., vital signs, wound dressing changes, care plans, etc.). under the general supervision of a registered nurse, medical provider, or supervisor.
3. Assist the medical provider and other medical staff with medical procedures and examination of residents during clinical exams within the CBRF.
4. Reinforce the teaching of patient/care treatment and follow-up done by a registered nurse or physician.
5. Provide resident support and ensure their safety.
6. Maintain complete and accurate medical records through the electronic health record system and/or paper chart appropriately (e.g., patient chart, labs, immunization record, active diagnoses).
7. Maintains proper care of the onsite provider office and sets up for treatments by checking supplies and equipment, cleaning room after each exam, sterilizing instruments. Perform an inventory of medical supplies and order as needed.
8. Actively participate in the CBRF's care coordination process including timely reporting residents' needs and changes in condition, attending resident and staff meetings, communicating effectively to the resident and/or health care representative, physician, other clinicians, management staff, and community case managers as applicable.
9. Interactions with residents that promote a caring environment. Supporting residents in a manner that promotes their dignity. Providing individualized care that encourages residents to attain or maintain physical, mental, and social well-being.
10. Observe and report changes in condition and progress of residents.
11. Ensure individual needs are met, including assistance with grooming, bathing, dressing, feeding, personal hygiene, walking, transferring, toileting, incontinent care, etc.
12. Maintain safe and clean environment.
13. Supervise resident/patient care activity performance by nursing assistants and other care attendants.
14. Able to use necessary supplies and equipment (oxygen, medication cart, etc.).
15. Assist nurse/management in Individualized Service Plan development.
16. Assist leadership in scheduling patient appointments as needed.
17. Be a representative of the CBRF in the community.
18. Be aware of current policies and procedures and communicate any needs for review or revision.
19. Plan and maintain own continuous professional and technical knowledge and growth through participation in professional, educational, and technical programs pertinent to area of interest.
20. Must be reliable and prompt when reporting to work.
21. Required to attend job related, in-service, meetings, and training to maintain professional and technical knowledge.
22. Must adhere to tribal law and other applicable laws as well as tribal personnel policies and procedures.

QUALIFICATIONS:
1. Graduate from an accredited school of nursing.
2. Current Licensure to practice as a Licensed Practical Nurse or a Registered Nurse in Wisconsin.
3. One-year clinical experience preferred.
4. Basic life support certification required.
5. Experience working with paper and electronic medical record systems preferred.
6. Able to demonstrate ability to utilize safe and proper lifting techniques.
