LPN Visit Nurse

Position Overview:

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes.

Key Responsibilities

• Deliver high-quality clinical services within the scope of practice, following physician plans of care and infection control protocols.
• Document patient visits thoroughly in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and care team members.

• Current and unrestricted LPN licensure, CPR certification, and a valid driver’s license with reliable transportation.
• Ability to work independently and adapt to diverse home environments.
• Preferred:
  Home care experience, strong communication, organizational skills, and flexibility to meet client needs.
