You will support a Long-Term Care (LTC) setting, providing skilled nursing care on a rotating schedule that includes day and evening shifts. • Provide direct nursing care to residents, communicating significant changes in condition promptly to physicians and families
• Complete applicable resident assessments and document findings thoroughly
• Administer medications following the 5 Rights and maintain accurate medication records
• Perform wound care and other treatments as prescribed; monitor residents for changes in condition
• Support CNAs by assisting with call lights and coordinating on-the-floor care
• Maintain accurate charting and ensure compliance with policies; elevate concerns as needed to leadership
• Assume leadership on the floor when needed, directing care and coordinating interdisciplinary team activities

• Licensure:
  Wisconsin Registered Nurse (RN) license, active
• Education:
  
  Two-year nursing degree required; four-year degree preferred
• Certifications:
  
  Current CPR/BLS certification
• Experience:
  
  1+ year of related nursing experience preferred
• Skills/ability:
  Ability to perform duties in a LTC/nursing home setting with accurate, detail-oriented charting and documentation
