• Administer prescribed medications and treatments according to physician orders and facility policy
• Monitor residents’ health status, document observations, and promptly report changes
• Provide direct bedside care and assist with activities of daily living (ADLs) as needed
• Assist with wound care, routine nursing procedures, and infection control practices
• Collaborate with the interdisciplinary care team to update plans and communicate changes
• Maintain accurate documentation in the electronic health record and ensure resident privacy and safety

• Active LPN license (state-appropriate licensure)
• Graduation from an accredited practical nursing program
• Prior experience in long-term care or assisted living preferred
• Strong communication and teamwork skills
• Ability to work in a fast-paced, resident-centered care environment
• Knowledge of safety standards and accurate documentation practices
