LVN​/LPN – LTC​/SNF

Job Summary:

The Licensed Practical Nurse (LPN) provides direct patient care, administers medications, and collaborates with the healthcare team to ensure residents' well-being. They play a vital role in monitoring patient conditions and maintaining accurate communication with families and physicians.

Responsibilities:

• Administer medications to residents following the 5 Rights to ensure safety and accuracy.
• Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
• Communicate changes in residents' conditions promptly to the charge nurse.
• Assist CNAs with answering call lights and supporting resident needs.
• Update physicians, family members, and healthcare team members regarding residents' status.
• Perform additional duties as assigned to support resident care and facility operations.

Qualifications:

• Hold and maintain an active Wisconsin LPN license.
• Maintain current CPR/BLS certification.
• Possess at least 1 year of related work experience preferred.
• Demonstrate exceptional accuracy, attention to detail, and strong communication skills.
• Be capable of working effectively within a team and handling clinical responsibilities with professionalism.
