LPN Visit Nurse

Role Overview :

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes.

Under supervision, you will deliver high-quality clinical services, document patient visits thoroughly, and participate in care coordination activities to ensure optimal patient outcomes.

• Provide nursing care within the scope of practice, following the care plan and infection control procedures.
• Accurately document patient visits in electronic medical records and visit verification systems.
• Report patient needs and changes in condition promptly, participate in case conferences, and communicate effectively with patients, families, and healthcare team members.

• Current, unrestricted Licensed Practical Nurse licensure in the practicing state.
• CPR certification, valid driver’s license, and reliable transportation.
• Ability to work independently in diverse home environments regardless of patient background.

• Home care experience, strong communication and organizational skills, and flexibility to meet patient needs.
• Ability to manage multiple tasks and work independently.
