RN Visit Nurse

Role Overview:

The Registered Nurse (RN) provides and oversees nursing care in patients' homes, ensuring compliance with laws, regulations, and agency policies. They coordinate care with healthcare professionals, patients, families, and community resources to maintain continuity of care.

• Deliver high-quality clinical services within scope of practice and infection control standards
• Coordinate care from admission to discharge with the care team
• Conduct clinical assessments according to program requirements and payer needs
• Verify patient eligibility and medical necessity for services
• Develop and update individualized care plans in collaboration with community providers
• Adjust care plans as needed with physician approval to meet patient needs

• Unrestricted RN licensure in the practicing state
• Valid driver's license, vehicle insurance, and access to reliable transportation
• Ability to work effectively in diverse home environments
• Preferred: 1+ years of RN experience, independence, strong communication, organizational skills
