Transitions at Home is seeking an experienced and motivated Registered Nurse (RN) Clinical
Administrator to lead the clinical and day-to-day operations of our Home Health Agency. This leadership position is ideal for an RN who is passionate about quality patient care, developing strong clinical teams, and helping a home health agency grow and succeed
The RN Clinical Administrator provides oversight of clinical services, staff performance, regulatory compliance, quality outcomes, and agency operations while ensuring patients receive safe, effective, and patient-centered care.

The successful candidate will be comfortable balancing clinical leadership with the operational responsibilities of running a successful home health agency.

Key Responsibilities- Provide leadership and oversight of home health clinical operations and staff.
- Ensure compliance with Wisconsin Home Health Agency regulations, CMS Conditions of Participation, and agency policies.
- Oversee patient referrals, admissions, plans of care, recertification's, transfers, and discharges.
- Supervise and support RNs, LPNs, Home Health Aides, therapists, and other clinical personnel.
- Monitor clinical documentation, patient outcomes, hospitalizations, and quality indicators.
- Lead QAPI, survey readiness, compliance, and performance improvement activities.
- Recruit, orient, coach, evaluate, and develop clinical staff.
- Monitor staffing, scheduling, productivity, and appropriate utilization of agency resources.
- Address patient complaints, clinical concerns, and employee performance issues.
- Collaborate with physicians, referral sources, community partners, and agency leadership.
- Support census growth, referral conversion, employee retention, and overall agency performance.
- Promote a culture of accountability, teamwork, quality, and exceptional patient care.
Office Hours Monday through Friday 8:00 am - 4:30pm

Current, unrestricted Wisconsin Registered Nurse (RN) license.
Meets applicable Wisconsin requirements to serve as a Home Health Agency Administrator.
Home health clinical experience is strongly preferred.
Previous nursing leadership, management, or supervisory experience preferred.
Knowledge of Medicare Home Health Conditions of Participation and Wisconsin home health regulations preferred.
Strong leadership, communication, organizational, and problem-solving skills.
Ability to manage clinical quality while supporting staffing, operational, and business goals.
BSN preferred but not required based on applicable experience and qualifications.
