Location: Elmwood ParkTo provide case management services for youth involved in the juvenile justice system, develop and maintain relationship with court and legal system, document casework activities and provide evaluation, advocacy and follow up to youth and their families throughout their court involvement or during the terms of their informal dispositions.
Review and analyze information relating to child or caregiver's social, psychiatric and medical history to learn the nature of the functional limitations as well as strengths each child and family member present.Review law enforcement referrals and provide recommendations to the District Attorney for disposition.Obtain and assess relevant information regarding child's development, i.e., social, familial, medical and vocational strengths.Conduct family and individual risk and needs assessments as well as continually assess the safety of youth on the caseload.Provide youth and family with appropriate information regarding services offered by the Human Services Department or the community.Conduct a thorough assessment of youth's strengths, needs and individual and family resources. Provide the Juvenile Court with recommendations regarding services, treatment and disposition.Develop a treatment plan in conjunction, consultation, and cooperation with the youth's family and all other involved in the youth's life in accordance with statutory guidelines. This treatment plan should outline the services to be provided by the Human Services Department and contracted agencies and set reasonable goals and time frames.Monitor the youth's compliance with the established court order; review and revise permanency plans as necessary and provide crisis services including emergency custody and changes of placement.Refer to and coordinate the various services to implement the treatment plan.Produce accurate and timely documentation and reports including but not limited to assessments, treatments plans, progress reports and court reports in order to insure compliance with federal and state requirements; maintain necessary e-WISACWIS documentation supporting these activities.Recommend to management changes or gaps in the system that prevent the delivery of services.Prioritize and organize time to cover assigned area and manage caseload in equitable manner.Maintain knowledge of pertinent departmental, state and federal policies, rules and regulations including state statutes 48 and 938, Access and Ongoing standards.Maintain contacts with the Juvenile Court, District Attorney, Public Defender and legal community that deal with and serve the youth population.Consult with other professionals in the field and in related fields to increase individual knowledge and understanding of various disciplines, changes and developments in the field of Human Services.Serve as liaison to the courts for the Human Services Department.Provide appropriate linking mechanisms as indicated by the court disposition.Supervision Received
Receives general supervision from the Supervisor of the Youth Justice Unit.
- Bachelor's degree in a Human Services field (Child development, criminal justice, educational psychology, vocational rehabilitation, guidance and counseling, psychology, social welfare, social work, or sociology).
- Experience working with at-risk youth, preferably in a juvenile justice or social services setting.
- Valid Wisconsin driver's license for required travel. Personal automobile, if used for work-related travel, must be covered by an automobile insurance policy.
- Prompt and regular attendance.
- Proficient in using case management software, Microsoft Office Suite and other online tools.
- Ability to work flexible hours as needed.
- Experience using WISACWIS preferred.
- Knowledge of Wisconsin State Statutes Chapters 48 and 938, and Access and Ongoing standards.
- Knowledge of fundamental social work principles.
- Knowledge of community resources.
- Strong understanding of trauma-informed care, restorative justice, and rehabilitation principles.
- Ability to deliver concise oral and written reports.
- Excellent communication, crisis intervention and problem solving skills.
- Ability to work well with other agencies.
- Ability to respond to crises and to make appropriate judgments as to resolution.
