Job DescriptionPurpose of Position:
Social Workers/Case Workers serve families involved in the Children, Youth, and Families Services team within Clark County Human Services. The goal of this position is to assess individual and family strengths and needs and provide intervention when necessary.
Essential Duties and Responsibilities:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may also be required and assigned. Certain roles within this position may involve program-specific responsibilities. These duties are defined in separate program guidelines or supplemental materials and are assigned as applicable.
- Actively engage clients, their families, and natural support in identifying and resolving challenges.
- Promote meaningful participation from clients and their support networks throughout intervention.
- Support the preservation and strengthening of family units and help prevent unnecessary out-of-home placements.
- Conduct investigations and comprehensive assessments to identify strengths, needs, and develop goal-oriented case plans in accordance with the rules, laws, and regulations of assigned program area.
- Maintain accurate, timely, and complete case records, including assessments, reports, and other required documentation in accordance with Clark County, State of Wisconsin regulations, Wisconsin Statues, and pertinent Federal Laws.
- Communicate clearly, concisely, and professionally – both verbally and in writing- with clients, families, colleagues, external agencies, and the Courts.
- Facilitate collaboration with schools, courts, health care providers, law enforcement, and other community partners to address needs and develop a comprehensive system of care for individuals and families.
- Educate and connect individuals and families with available community resources and empower them to access services independently to promote long-term success.
- Refer clients to appropriate community-based resources and ensure coordinated service delivery.
- Foster and maintain professional, respectful, and effective working relationships with clients, colleagues, supervisors, external agencies, county departments, and the general public.
- Participate in on-call rotations to meet statutory requirements outside of regular business hours.
- Participate in required trainings, staff meetings, professional development opportunities and complete continuing educational requirements to enhance competency and service delivery.
Minimum Training and Experience Required to Perform Essential Job Functions A Bachelor's degree in Social Work or approved equivalent Bachelor's degree in a related Human Services program from an accredited college or university. Preference will be given to candidates credentialed as a Certified Social Worker in the State of Wisconsin. Must possess a valid driver's license and own a reliable vehicle with automobile liability insurance that meets the minimum county coverage to perform off-site responsibilities.
Must be able to successfully pass caregiver and criminal background check.
