Health Care Administrators Association® (HCAA) seeks an IRIS Consultant to support individuals in maintaining independence through the IRIS program in Wisconsin. You will build trust with participants and help develop IRIS Plans, conducting regular contacts and visits to ensure eligibility and service options.
Ideal candidates have a bachelor’s degree in social work or related fields, or extensive social services experience, with bilingual skills a plus.
