Role Overview:
An IRIS Consultant supports adults, older adults, and individuals with disabilities in Wisconsin's IRIS program.
Key Responsibilities:Build trusted relationships with participants, develop and update personalized IRIS Support Plans, conduct monthly contacts and quarterly in-person visits, promote self-determination and community inclusion, monitor health and safety, educate on program eligibility, collaborate with community resources, maintain accurate documentation, and assist with assessments.
Qualifications:Bachelor's degree in Social Work or related field with at least one year of experience, or four+ years of relevant direct service experience, or current IRIS Consultant status.
PreferredSkills:
Case management, care coordination, disability services, long-term care, social work, community-based services, healthcare navigation, and cultural competency. Bilingual skills in languages such as Spanish, Hmong, Arabic, Russian, or ASL are encouraged.
Additional Requirements:Strong communication skills, ability to work independently, proficiency with technology, reliable transportation, valid driver's license, and willingness to travel within the region.
