Location: North Lake
Role:
IRIS Consultant, a full-time position supporting adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.
Responsibilities:- Build trusted relationships with participants
- develop and update individualized support plans
- conduct regular contact and in-person visits
- promote self-direction and independence
- monitor health and safety
- educate about program resources
- collaborate with community partners
- maintain accurate documentation
- assist with functional screens.
- Bachelor's in Social Work or related field with at least one year of experience
- four+ years of direct service experience
- current IRIS Consultant in good standing.
Preferred skills include case management, care coordination, disability services, and healthcare navigation.
Strong communication, cultural awareness, and ability to work independently are essential.
Additional skills:- Bilingual abilities (e.g., Spanish, Hmong, ASL)
