• Requirements:
  Qualified applicants MUST have at least 1 year of experience in a Medicare-certified hospice agency, a valid RN license, and be willing to obtain MA licensure.

• Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
• Provide skilled nursing care in patients' homes, including medication management, wound care, and infusion therapy.
• Educate patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence.
• Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes.
• Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care.
