• Provide direct patient care, including administering medications, performing vital signs, and monitoring resident conditions
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Participate in resident assessments, care planning, and discharge planning
• Collaborate with other healthcare professionals to ensure seamless continuity of care
• Maintain accurate and detailed records of patient care and treatment
• Educate residents and their families on treatment options, medications, and disease management

• Current LPN license in the state of Massachusetts

• Ability to work effectively in a fast-paced environment with minimal supervision
• Excellent communication and interpersonal skills
• Ability to lift, transfer, and reposition patients with assistance as needed
• Certification in Basic Life Support (BLS) or within 30 days of hire
