MDS Coordinator - Woburn, Massachusetts

As an MDS Coordinator at Woburn Rehabilitation and Nursing Center, your responsibilities will include:
• Conduct and coordinate the development and completion of the resident assessment
• Conduct resident assessments in accordance with current rules, regulations, and guidelines
• Implement resident CAA's
• Participate in facility survey readiness and throughout the survey
• Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
Requirements for the MDS Coordinator position:
• Current unencumbered State of Massachusetts RN or LPN License
• Two years of MDS and PPS experience
• Care Planning experience
