Occupational Therapist- Home Health, Senior Living

BAYADA Home Health Care is currently seeking an experienced occupational therapist (OT) for a full time or per diem opportunity performing home health visits. This office works with adult and geriatric patients on a per visit basis in senior living communities throughout Somerset, Middlesex and Monmouth Counties
.

As a home care Occupational Therapist, I will be an integral member of a multi‑disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

• Perform outpatient therapy evaluations in order to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities.
• Professionally operate at the top of licensure utilizing current research‑based approaches to care in order to maximize functional independence for clients.
• Work closely with home health therapy partners to appropriately transition care from home health to outpatient services to achieve client‑centered goals in their living environment, allowing the right care at the right time for each client.
• Educate the client, family and community caregivers on the care plans created for clients in order to achieve a team approach to maximizing functional independence.
• Perform evaluations, treatments and proper discharge planning for clients utilizing an outpatient EMR platform to deliver and document individualized treatment approaches to meet the needs of clients.

• Current NJ Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Base Pay, depending upon qualifications and subject to negotiation: $60.00 - $68.00 per point
