BAYADA Home Health Care is seeking a Home Health Physical Therapist for a position in Colonia, New Jersey.

As a Home Health Physical Therapist, you will be an integral member of a multidisciplinary health care team, providing rehabilitative care and skilled nursing to adult and senior clients throughout Middlesex County.

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength & endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance interventions, and other modalities.
• Continuously assess and revise the PT care plan, and collaborate with nursing on multidisciplinary care plans as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote the client’s health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

• Current New Jersey Physical Therapist - PT license.
• Minimum of one year of recent work experience as a Licensed Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Base Pay: $65.00 – $75.00 per point (subject to negotiation).

$20,000 Sign on Bonus! for full‑time PTs.
