BAYADA Home Health Care is currently seeking an experienced Occupational Therapist (OT) for a full time or per diem opportunity performing home health visits. This office works with adult and geriatric patients in senior living communities throughout Somerset, Middlesex and Monmouth Counties. As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

• Perform outpatient therapy evaluations in order to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities.
• Professionally operate at top of licensure utilizing current research-based approaches to care to maximize functional independence for clients.
• Work closely with home health therapy partners to appropriately transition care from home health to outpatient services to achieve client-centered goals in the living environment.
• Educate the client, family and community caregivers on the care plans to achieve a team approach and maximize functional independence.
• Perform evaluations, treatments and proper discharge planning using the outpatient EMR platform to document and deliver individualized treatment approaches.

• A current New Jersey Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the AMA/ACHE and AOTA (as indicated by school transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

• Base Pay, depending upon qualifications and subject to negotiation: $60.00 - $68.00 per hour
• Additional information about sign-on bonus: $10,000 sign-on bonus for full-time OT's.
