• Assessment:
  Collect comprehensive data, perform systematic assessment, recognize clinical changes, identify education needs.
• Plan:
  Develop patient/family‑centered care plan, prioritize, coordinate continuum of care.
• Implementation:
  Communicate and implement plan, provide education, document electronically, administer medication, coordinate multidisciplinary discharge.
• Evaluation:
  Evaluate progress, use critical thinking, initiate interventions for changes.
• Additional:
  Identify ethical issues, enhance patient satisfaction, maintain certifications, adhere to ANAC and HMH competencies, lift 35 lbs., push/pull 10 lbs., stand minimum 6 hours.

• BSN in progress or willing to complete within 3 years of hire.
• Strong learning ability, accountability, teamwork, dedication to education, patient focus.
• National certification in specialty preferred.
• NJ State Registered Nurse License.
• BLS/CPR certification within 60 days of hire.

Minimum rate: $42.07 hourly. Compensation may vary based on market data, experience, education, skills, location, internal equity, and budget.
