• Provide professional nursing care to patients in their home.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Care for patients and their significant others in a holistic manner.
• Demonstrate knowledge and competency in assigned treatments and procedures.
• Utilize interdisciplinary team and document all activities accurately.
• Maintain compliance with payor source guidelines and authorizations.
• Complete nursing assessments and collaborate with physicians on care plans.
• Implement teaching strategies to meet established goals.
• Act as clinical supervisor of para-professional home health aide staff.

• Associate's Degree in Nursing or Diploma Certificate in Nursing required
• BSN preferred

• At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice

• Current NJ State Professional Registered Nurse License
• AHA Basic Health Care Life Support HCP Certification required
