About This Role

The Clinical Care Coordinator RN in Home Care at Hackensack Meridian Health is responsible for providing telephonic case management and coordinating care for patients in need of skilled nursing services. This role is ideal for experienced RNs who excel in communication and collaboration within a healthcare team.

• Provide telephonic case management and assist in clinical operations.
• Collaborate with specialists and healthcare professionals to deliver home health services.
• Ensure effective communication and completion of treatments/procedures.
• Utilize the nursing process to provide nursing visits and manage a caseload.
• Teach patients and families about their individualized plan of care.
• Administer medications, treatments, and procedures safely and effectively.
• Assist client care coordinators in prioritizing patient visits.
• Function as a patient advocate and perform crisis intervention as needed.
• Identify community resources to meet patient needs and refer patients accordingly.
• Document assessment data, patient care activities, and interventions.

Education:

• Associate's Degree in Nursing or Diploma Certificate in Nursing required

Experience:

• Minimum of 2 years RN experience required
• Minimum of 3 years certified home care experience preferred

Licenses &

Certifications:

• Current NJ State Professional Registered Nurse License required
• BLS AHA Basic Health Care Life Support HCP Certification required
• OASIS certification preferred
• BSN preferred
