- Provide professional nursing care to patients in their home.
- Assess, develop, implement, and evaluate home nursing care needs.
- Ensure compliance with physician orders and documentation.
- Demonstrate holistic care for patients and their significant others.
- Complete nursing assessments and medication reconciliations.
- Collaborate with physicians on the Plan of Care.
- Act as a liaison among physicians, management, and healthcare team members.
- Supervise para-professional home health aide staff.
- Maintain awareness of new standards and incorporate them into practice.
- Complete routine assignments independently and meet productivity requirements.
Education:
- Associate's Degree in Nursing or Diploma Certificate in Nursing
- At least 6 months RN experience or 1 year LPN experience, preferably in home care or hospice
Certifications:
- Current NJ State Professional Registered Nurse License
- AHA Basic Health Care Life Support HCP Certification
