• Candidates must hold an active New Jersey Registered Nurse (RN) license
  .

• Current BLS/CPR certification is required.

• Strong clinical assessment, medication administration, and documentation skills are required.

• Experience with
  EHR or digital documentation systems is required.

• Excellent communication, patient education, and care coordination skills are required.

• A valid driver's license and reliable transportation are required.

• Experience in home health, hospice, or community nursing is preferred.

• A BSN or equivalent education is a plus.

• The consultant will provide and oversee high-quality, patient-centered care in home settings
  .

• The consultant will conduct clinical assessments and develop individualized care plans.

• The consultant will coordinate patient care with physicians and therapists
  .

• The consultant will educate patients and their families regarding care.

• The consultant will perform skilled nursing procedures as required.

• The consultant will support regulatory compliance and quality improvement initiatives
  .

• The consultant will travel within New Jersey, as required
