Nurse Practitioner​/Physician Assistant - Primary Care

Position Overview

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Woodbury, MN.

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full‑time providers care for patients Monday–Thursday; option for Fridays to be a clinical or administrative day.
• Manage a panel of approximately 200 patients (12–16 patients per day).
• Design and manage your schedule.
• Develop and implement individualized care plans appropriate to patients and their desires.
• Receive clinical support from Clinical Assistants and care coordination teams including LPNs, RNs, and social workers.
• Access diagnostics such as labs and X‑rays as needed.
• Collaborate with community partners within home‑care, hospice, and other resources as appropriate.

• Graduate of an accredited Nurse Practitioner or Physician Assistant education program.
• Board certified or board‑prepared through AANP, ANCC, or NCCPA.
• Licensed or able to obtain licensure through the state Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the required state of work.
• Strong verbal communication skills and excellent interpersonal and customer‑service skills.
• Valid, unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required.

• Minimum 2 years of clinical experience as a Nurse Practitioner or Physician Assistant.

• Base salary of $125,000 to $170,000 USD, plus quality/productivity incentives.
