POSITION TITLE: MnCHOICES Assessor

DEPARTMENT: Health & Human Services

POSITION STATUS: TEMPORARY

The MnCHOICES Assessor is responsible for conducting person-centered assessments to determine eligibility for long-term services and supports under Minnesota’s MnCHOICES Assessment Program. The position ensures compliance with state and federal guidelines and works collaboratively with individuals, families, and community partners to develop care plans that promote independence, health, and well-being. Work is performed under the supervision of Social Services Supervisor.

MnCHOICES Assessment Services
  • Provide assessment services to qualifying individuals under the MnCHOICES framework.
  • Obtain and maintain eligibility as a certified MnCHOICES Assessor.
  • Complete and maintain certification and re-certification as required by the Minnesota Department of Human Services (DHS).
  • Conduct assessments by meeting with individuals and/or their representatives to gather information through conversational interviews, site visits, and consultations with other agencies or professionals.
  • Determine program eligibility using assessment data to establish eligibility for programs such as Home and Community-Based Services (AC, BI, CAC, CADI, DD, EW) and Community First Services and Supports (CFSS).
  • Develop person-centered care plans based on assessment findings that identify specific needs, goals, supports, and next steps.
  • Provide referrals to appropriate community resources, agencies, and professionals for services not covered by the assessment.
  • Coordinate long-term services and supports to ensure they are appropriate, cost-effective, and aligned with individual goals.
  • Stay current on state and federal laws, regulations, eligibility criteria, and available resources related to MnCHOICES and long-term care programs.
  • Follow all State and Centers for Medicare & Medicaid Services (CMS) guidelines related to the delivery of Medicaid or state-offered services to persons with disabilities and older adults.
  • Ensure compliance with all state and federal requirements governing the delivery of Medicaid and state-funded services.
Minimum Qualifications
  • Bachelor’s degree in Social Work, Nursing, Psychology, Human Services, or a related field.
  • Completion of MnCHOICES Assessor certification training (or ability to obtain certification within required timeframe).
  • Valid Minnesota driver’s license and access to reliable transportation for travel to client sites.
Preferred Qualifications
  • Experience conducting MnCHOICES assessments or other long-term care assessments.
  • Knowledge of community-based resources, waiver programs, and county human service systems.
  • Experience working with individuals with disabilities, older adults, or vulnerable populations.
  • Proficiency in documentation systems such as SSIS, MMIS, or other case management platforms.
Knowledge, Skills, and Abilities
  • Strong understanding of person-centered planning principles and service coordination.
  • Ability to interpret and apply complex state and federal program regulations.
  • Excellent communication, interviewing, and interpersonal skills.
  • Ability to maintain confidentiality and exercise sound professional judgment.
  • Strong organizational and documentation skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Hybrid-ork performed in office, community, and home settings.
  • Regular travel within the county/region required.
