The Registered Nurse will provide professional nursing care to patients in their homes, focusing on assessment, development, implementation, and evaluation of individual care needs. This role is ideal for nurses seeking a meaningful connection with patients in a home health setting.

• Provide professional nursing care to patients in their home.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Demonstrate holistic care for patients and their significant others.
• Complete nursing assessments and medication reconciliations.
• Collaborate with physicians on the Plan of Care.
• Update care plans within two weeks of changes in patient status.
• Act as a liaison among physicians, management, and healthcare team members.
• Supervise para-professional home health aide staff.
• Maintain awareness of new standards and incorporate them into practice.

Education:

• Associate's Degree in Nursing or Diploma Certificate in Nursing required
• BSN preferred

Experience:

• Minimum of six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice

Licenses &

Certifications:

• Current NJ State Professional Registered Nurse License required
• AHA Basic Health Care Life Support HCP Certification required
• IV Certification preferred
