• Provide professional nursing care to patients in their home.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Demonstrate holistic care for patients and significant others.
• Complete nursing assessments and medication reconciliations.
• Collaborate with physicians on the Plan of Care.
• Update care plans within two weeks of changes in patient status.
• Act as a liaison among physicians, management, and healthcare team members.
• Supervise para-professional home health aide staff.
• Maintain awareness of new standards and incorporate them into practice.

• Education: Associate's Degree in Nursing or Diploma Certificate in Nursing
• Experience: Minimum of six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice

  Current NJ State Professional Registered Nurse License; AHA Basic Health Care Life Support HCP Certification; IV Certification preferred
