Position SummaryThe Home Health Aide, reporting directly to the Director of Clinical Services, is responsible for providing compassionate, hands‑on care and supportive services to patients in the home setting. This role promotes the physical, personal, and emotional well‑being of patients while supporting the interdisciplinary team in achieving the goal of helping each patient reach their maximum level of self‑care and independence, when possible.
EssentialDuties

- Work in collaboration with the supervising RN, PT, or OT to fulfill the defined patient‑specific Plan of Care.
- Collaborate with the care team as an extension of nursing services to ensure all patient needs are fully addressed.
- Deliver hands‑on personal care and supportive services to assist with safe activities of daily living, in accordance with the established HHA Plan of Care, and in compliance with the applicable laws, regulations, and agency policies.
- Document care provided according to agency policy.
- Use Durable Medical Equipment safely.
- Participate in activities that promote patient safety, quality, and regulatory compliance.
- Participate in professional development and lifelong learning activities.
- Other duties as assigned.
Education and/or Experience
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required.
- Current Certified Nurse Aide (CNA) certification or Home Health Aide (HHA) certification
- Current Basic Life Support/CPR certification
- Preferred home care experience
- Current and valid driver’s license, reliable transportation, and current liability auto insurance.
- Ability to work well with others and take direction from management, as well as take initiative – willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
- Self‑motivated to accomplish identified goals with a strong sense of accountability for results.
- Experience with Microsoft Office Suite.

- Frequent travel within assigned service area is required.
- While performing the duties of this job, the employee is occasionally required to stand for extended periods of time, move, communicate, and identify written information. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to fifty (50) pounds independently, and occasionally lift and/or move up to two hundred (200) pounds with assistance.
- Work for this position is performed in patient homes, care settings/facilities, and office settings. This position will have to navigate through a variety of different settings to provide care.
- There is exposure to varied home environments and potential infectious conditions.
- Must comply with all Dover Health health and safety requirements.
- This role involves actively participating in all staff activities aimed at fostering teamwork, unity, and morale. The individual will contribute as a collaborative team player, working alongside colleagues to create a supportive and cohesive work environment.
