Driver files:

• For new drivers:
  • Make sure driver completes all necessary paperwork
  • Follow FMCSA guidelines to contact previous employer regarding driving record and testing results
  • Copy of driver record from state agency
  • Send driver for initial drug screen
  • Review DMP rules, code of conduct and drug policy – have them sign acknowledgement
  • Obtain copy of medical card and driver’s license
  • Prior to first drive, send for alcohol screen
  • DMP driver agreements and substance abuse policy
• For current drivers:
  • Annual declaration of violations (to be completed by driver)
  • Obtain a copy of driving record from state agency (every year)
  • Send drivers for randomly selected drug and alcohol screens
• For terminated drivers:
  • Document files with termination paperwork
  • Remove them from testing pool
• Other eligibility requirements:
  • Maintain a calendar of expiration dates for current driver medical cards and licenses
  • Maintain records of random drug and alcohol testing, as well as results
• Insurance:
  • Communicate with Mary regarding drivers to be added or removed from DMP insurance (secure appropriate info from drivers if needed)
  • Notify of any accidents that may need to be reported

• Maintain records of driver training (dates and attendance)
• Conduct mock audits (when needed)
