Rocky Mountain Care is looking for a full-time Intake/Admissions Nurse (RN/LPN) Mon-Friday 9a-6p, One weekend every 5-7 weeks. Rotating on call every 5-6 weeks.

• Facilitate admissions for incoming Skilled Nursing patients (does not include direct patient care)
• Verify and enter physician orders
• Identify level of care needed
• Receive and give verbal orders accurately
• Review clinical documentation to determine Skilled Nursing placement
• Exceptional interpersonal and customer service skills
• Ability to communicate clearly with medical providers
• Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
• Manage after-hours and weekend referrals (rotating on call)

• Current license as a licensed LPN/RN from the Utah State Board of Nursing
• 1-year Skilled nursing facility experience
• Good verbal and written communication skills
• Basic computer knowledge
• Ability to effectively problem solve
• Excellent customer service skills
• Knowledge of Medicare/Medicaid Skilled Nursing placement criteria preferred
• Knowledge of Medicare/Medicaid home care and hospice criteria preferred
• Knowledge of Medicare, Medicaid and 10A criteria

Standard shift:
Monday - Friday 9am-6p. One schedule weekend every 5-7 weeks. Rotating on-call approximately every 5-6 weeks. Day shift.
