Provides certified nursing assistant services to assigned residents in accordance with care plans, facility policies, procedures, and the direction of supervisors.
Required Qualifications- Certified Nursing Assistant in good standing with the state (preferred).
- High school diploma or equivalent.
- Current CPR/BLS certification.
- Current Food Handler Permit.
- No disciplinary action in effect against certification.
- At least one year of experience as a certified nursing assistant in long‑term care (preferred).
- Provides supportive services to nurse, med‑tech, and other staff as needed.
- Assists residents with activities of daily living (ADLs) in accordance with care plans.
- Assists with lifting, turning, moving, positioning, and transporting residents using proper body mechanics.
- Coordinates dining room services at assigned mealtimes, including set‑up, clean‑up, meal tray delivery, feeding assistance, and documentation of meal intake.
- Delivers nutritional supplements and provides assistance to ensure intake, documenting intake accordingly.
- Weighs residents as per facility policy, records weights, and reports weight changes to nurse and supervisor.
- Tracks the condition of residents’ skin, reporting pressure areas, skin breakdown, or skin tears.
- Assists nursing staff with toileting program activities.
- Completes flow sheets daily to document specified tasks.
- Keeps nurse and supervisor informed of factors that interfere with performing assigned work.
- Reports changes in a resident’s condition to the resident’s nurse and supervisor.
- Treats all residents with dignity and respect; promotes and protects their rights.
- Adheres to all facility policies, procedures, and regulatory guidelines.
- Follows safety and hygiene measures at all times.
- Makes and maintains the confidentiality of protected health information.
- Reports noncompliance, abuse, neglect, or retaliation to appropriate personnel.
- Reports occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policy.
- Participates in life‑safety and emergency drills and training; fulfills responsibilities during emergency activation.
- Reports work‑related injuries and illnesses immediately to supervisor.
- Follows infection control policies and procedures.
- Completes all assigned training and competency assessments.
- Attends care‑plan meetings of assigned residents.
- Participates in QAPI or facility assessment activities as needed.
- Accepts certified nursing assistant assignments as staffing needs require.
- Uses proper lifting and body mechanics while delivering care.
Physical Requirements
- Proficient in reading, writing, and speaking English.
- Supportive team member with strong interpersonal skills.
- Capable of making independent decisions when warranted.
- Displays tact and patience with residents, families, staff, and visitors.
- Can relocate information about resident conditions effectively.
- Maintains health and physical fitness; meets general health requirements per facility policy.
- Available to work beyond normal hours, including weekends and holidays.
- Capable of assisting in evacuation during emergencies.
- Physical abilities: bend, stoop, kneel, crouch, perform overhead lifting, and move residents; lift or move a minimum of 35 pounds.
- May be exposed to falls, burns, irritants, infectious waste, diseases, or hostile interactions.
- Basic computer skills and ability to navigate electronic medical record systems.
Adherence to facility standards, policies, and the compliance and ethics program is a condition of employment and will be considered during performance evaluations. Violations may result in disciplinary action, up to and including termination. Performance will be evaluated on a scale ranging from "Unsatisfactory" to "Exceeds Standards."
