Rocky Mountain Home Health & Hospice-Davis Weber in Woods Cross, UT is seeking an experienced registered nurse to plan, organize and direct hospice care in patients' homes and communities.
The RN will conduct assessments, develop care plans with patient and family, administer medications, educate families, and coordinate with physicians and the interdisciplinary team.
A state nursing license, CPR, and a valid driver’s license are required; hospice/home health experience is preferred.
