Matrix Medical Network is expanding its in-home health assessment program. This position performs comprehensive health assessments in patients' homes or SNFs, reviewing medical history, adherence, and social factors to close care gaps.
You will work closely with physicians, case managers, and care teams to deliver exceptional, patient-centered care. Eligible candidates are licensed NPs or PAs with current credentials, board certification, and BLS/ACLS.
