Rapid Re-Housing Case Manager
The Rapid Re-Housing Case Manager is responsible for securing and placing survivors of domestic violence, dating violence, human trafficking, sexual assault, and stalking into transitional housing or providing rental assistance through the Transitional Housing Rapid Rehousing program. The role supports survivors in regaining empowerment and self-sufficiency, working closely with landlords in Lake County, Illinois, and partnering with A Safe Place staff, community leaders, and housing agencies.
The position is 24/7 on call and reports to the Associate Director of Crisis Housing.
- Develop partnerships with landlords and educate them on safety policies.
- Provide information on the Illinois Domestic Violence Act, Orders of Protection, Human Trafficking, and stalking laws to enhance client safety.
- Manage client application processes, income updates, household changes, and re‑certifications to maintain stable housing.
- Oversee temporary housing for two families in units leased by A Safe Place.
- Conduct outreach to community-based organizations, housing resources, and landlord associations to identify new housing opportunities.
- Coordinate intake assessments and eligibility evaluations.
- Complete housing assessments, housing summaries, and VISPDAT evaluations.
- Create service plans and deliver employment training, education, economic assistance, counseling, children’s services, legal advocacy, and refer for additional services.
- Implement client assessment tools and comply with reporting requirements, including funder‑prescribed evaluations.
- Maintain accurate, timely documentation in client records per agency policies and funding requirements.
- Participate in a rotating on‑call schedule (seven days) and work one weekend per 4–6 weeks.
- Attend Lake County Homeless Coalition coordinated entry meetings and other required meetings.
- Collaborate with A Safe Place staff and external partners to refer and support additional services.
- Recruit and develop relationships with community and corporate partners, including providing training.
- Attend and participate in all team and staff meetings.
- Act as a team player, conduct training and education as requested, and perform additional responsibilities as assigned.
- Minimum bachelor’s degree in Social Work, Counseling, Education, or a closely related field.
- Flexible schedule to meet client needs, respond to crisis, and support the Crisis Response team.
- Bilingual skills preferred.
- Ability to motivate clients, engage tenants in training and education, and support progress toward service plan and independence.
- Excellent written and oral communication skills; proven ability to complete documentation, develop group materials, and convey information clearly.
- Excellent interpersonal skills with genuine concern and respect for clients and staff.
- Ability to work independently and as part of a team, exercising mature judgment.
- Highly motivated self‑starter capable of coordinating multiple projects and tasks.
- Knowledge of handling confidential information.
- Ability to understand and sensitively relate to individuals from varied socio‑economic, racial, cultural, and experiential backgrounds.

A Safe Place is a recognized 501(c)(3) nonprofit organization listed under Lake County Crisis Center for the Prevention & Treatment of Domestic Violence Inc. This position ensures compliance with Medicaid billing requirements, documentation standards, regulatory expectations, and adherence with CARF standards and guidelines.
