Haven Home Health & Hospice is seeking a compassionate and dedicated PRN Hospice Medical Social Worker (MSW) to join our interdisciplinary team. In this role, you ll provide emotional, psychosocial, and resource support to hospice patients and their families while helping them navigate one of life s most important journeys.

What You ll Do- Complete psychosocial assessments and develop individualized plans of care.
- Provide counseling and emotional support to patients, families, and caregivers.
- Assist families with community resources, financial assistance programs, and advance care planning.
- Collaborate with nurses, physicians, chaplains, aides, and the interdisciplinary team to provide holistic patient care.
- Participate in weekly interdisciplinary team (IDT) meetings.
- Maintain timely, accurate documentation in accordance with hospice regulations.
- Support patients and families through grief, loss, and bereavement.
- Assist with discharge planning and community resource referrals as appropriate.
- Represent Haven professionally within the community and uphold our mission of compassionate hospice care.
- Bachelor s or Master s Degree in Social Work or Sociology from an accredited institution.
- Current Oklahoma Social Worker license or certification, as required.
- One (1) year of experience in hospice, home health, healthcare, or community-based services preferred.
- Excellent communication and interpersonal skills.
- Understanding of hospice philosophy and the end-of-life process.
- Valid Oklahoma s driver s license.
- Reliable transportation and current automobile liability insurance.
- Ability to travel throughout the assigned service area.
