As a Hospice LPN, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with physician orders and individualized plans of care.

Responsibilities include:

• Providing skilled nursing care to hospice patients in their homes and care facilities
• Monitoring and reporting changes in patient condition
• Assisting with symptom management and patient comfort
• Educating patients, families, and caregivers regarding care needs
• Collaborating with the interdisciplinary hospice team
• Maintaining accurate and timely clinical documentation
• Participating in on-call, holiday, and weekend rotations as assigned
• Supporting patients and families with professionalism, empathy, and respect

• Current Oklahoma Licensed Practical Nurse (LPN) license
• Hospice, home health, or related nursing experience preferred
• Strong clinical assessment and communication skills
• Ability to work independently while collaborating effectively with the care team
• Valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel throughout the service area
