The Hospice RN Field provides professional nursing care to home care patients as prescribed by the physician, ensuring compliance with the State Nurse Practice Act and agency policies and procedures. This role may also involve delivering nursing services in institutional settings while adhering to the respective policies and procedures.

Qualifications
  • Active Registered Nurse (RN) license in the State of Oklahoma.
  • At least 2 years of hospice nursing experience preferred.
  • Recent experience in direct patient care within a hospice or institutional setting is preferred.
  • Strong verbal, written, and electronic communication skills.
  • Ability to adapt to frequently changing tasks.
  • Valid driver’s license, reliable transportation, active auto liability insurance, and willingness to travel within the service area under varying weather conditions.
Responsibilities
  • Lead and actively participate in interdisciplinary team meetings and case discussions.
  • Manage care for up to 16 patients across different settings, ensuring that all patient needs and goals are met.
  • Utilize critical thinking, strong decision-making, and effective communication skills in patient care.
  • Develop and implement individualized care plans based on comprehensive assessments, updating them at least every 15 days or as patient needs evolve.
  • Perform a range of RN duties, including admissions, routine skilled nursing visits, evaluations, imminent visits, and on‑call or PRN visits.
  • Ensure compliance with all policies, procedures, and directives from leadership.
  • Supervise hospice aides by conducting visits at least every 14 days and addressing any concerns through direct observation and training.
  • Conduct monthly supervision of assigned LPNs, including patient home visits and clinical record reviews.
  • Provide support for all levels of hospice care, including routine, respite, continuous, inpatient, and imminent visits.
  • Maintain thorough and compliant documentation:
    • Accurate – Reflects sound clinical judgment.
    • Timely – Electronic documentation completed by 10:00 AM the following day (weekend notes by Monday at 10:00 AM).
    • Complete – Includes all required fields, narratives, and confirmations.
    • Acceptable – Meets Medicare standards as outlined in the Progressive Hospice Policy and Procedure Manual.
  • Attend interdisciplinary team meetings every 15 days, as well as case conferences and care coordination meetings with healthcare partners.
  • Keep personnel files up to date and respond promptly to emergency situations.
  • Communicate changes in patient conditions to the appropriate parties.
  • Coordinate care with facility staff, physicians, pharmacies, and other healthcare team members.
  • Provide counseling to patients and families to support their emotional and practical needs.
  • Educate hospice personnel on procedures and assist with the onboarding of new staff.
  • Uphold ethical standards and comply with laws governing hospice care and workplace regulations.
Organizational Relationship

Reports to Administrator or other assigned managers.

  • Have strong hearing ability to assess and respond quickly to situations.
