The Intake Coordinator at Elmwood Healthcare - Home Health will play a critical role in managing the intake process for new patients, ensuring a smooth and efficient transition into our home health care services. This position involves collaborating with patients, families, and healthcare professionals to gather necessary information, coordinate services, and facilitate a seamless onboarding experience.

Key Responsibilities:
- Patient Intake and Assessment: Conduct initial patient assessments to gather comprehensive information regarding medical history, current health status, and specific care needs.
- Verify insurance coverage and coordinate benefits to ensure eligibility for services.
Coordination and Communication:
- Act as the primary point of contact for patients, families, and referral sources, addressing inquiries and providing clear information about home health care services.
- Collaborate with medical professionals, including physicians, nurses, and therapists, to gather necessary documentation and information for care planning.
Documentation and Record-Keeping:
- Ensure accurate and timely completion of all intake forms, including patient consent, medical history, and insurance documentation.
- Maintain up-to-date patient records in compliance with confidentiality and privacy regulations.
Service Coordination:
- Schedule and coordinate initial home visits and care plans with appropriate healthcare providers.
- Facilitate the transition of care by ensuring that all necessary equipment, supplies, and resources are available to the patient.
Quality Assurance:
- Monitor and follow up on patient satisfaction and care quality to ensure compliance with company standards and regulatory requirements.
- Address and resolve any issues or concerns that arise during the intake process.
Administrative Support:
- Manage and prioritize a high volume of intake requests, ensuring timely processing and coordination.
- Assist with additional administrative tasks as needed, including data entry and reporting.
Other:
- Accepts only those patients whose needs can be met by the services it provides;
- Monitors and responds to electronic referrals via electronic referral systems
- Verifies and coordinates all insurance benefits of prospective patients (referrals);
- Creates, oversees and maintains the schedules of approximately 100 patients based on physician orders;
- Creates, oversees and maintains the schedules of designated team members (clinicians) in correlation to patient needs and staffing variances;
- Enters scheduling data to create and maintain accurate orders in accordance with care plans;
- Works inter-disciplinarily to create a consistent flow of high-quality patient care;
- Contacts care-providers, clinicians and patients with updates regarding day-to-day scheduling matters;
- Receives, processes and facilitates referrals in accordance with the systematic guidelines set forth by Elmwood Home Care;
- Accepts only those patients whose needs can be met by the services it provides;
- Monitors and responds to electronic referrals via electronic referral systems;
- Verifies and coordinates insurance benefits to safeguard reimbursement of services provided;
- Works closely with the sales team to process new referrals and disperse updates on pending referrals;
- Generates reports and data regarding scheduling, care coordination and staffing as designated;
- Maintains and updates patient status updates in correlation to provision of home-health services;
- Monitors and provides feedback on agency care coordination systems and processes;
- Acts as the bridge between all pre- and post-transitional care processes;
- Performs performance appraisal and review of department personnel;
- Provides counseling and/or disciplinary action to department personnel when indicated;
